HIPAA Notice of Privacy Practices
Last updated: October 22nd, 2026
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
Lux Family Chiropractic respects your privacy and is committed to protecting your health information.
This notice explains your rights, your choices, how we may use or share your health information, and our responsibilities under applicable privacy laws.
1. Your Rights
Access your health records
You have the right to inspect or request an electronic or paper copy of your medical records and other health information maintained by Lux Family Chiropractic, subject to applicable legal exceptions.
We will generally respond within 30 days, or sooner when required by applicable law. We may charge only fees permitted by law.
Request corrections
You may request that we amend health information you believe is incorrect or incomplete. We may deny a request when permitted by law, but we will explain the reason in writing and describe your available rights.
Request confidential communications
You may ask us to contact you in a particular way, such as by telephone, email, or mail at a specific address. We will accommodate reasonable requests.
Request restrictions
You may ask us to limit certain uses or disclosures of your health information for treatment, payment, or healthcare operations. We are not generally required to agree, except where the law requires otherwise.
If you pay for a service in full out of pocket, you may request that we not disclose information about that service to your health plan for payment or healthcare operations. We will honor that request unless disclosure is required by law.
Request an accounting of disclosures
You may request an accounting of certain disclosures of your health information during the previous six years, subject to applicable exclusions.
One accounting within a 12-month period is generally free. Additional requests may be subject to a reasonable, cost-based fee.
Receive a copy of this notice
You may request a paper copy of this notice at any time, even if you previously received it electronically.
Choose a personal representative
A person legally authorized to act on your behalf may exercise your privacy rights, subject to applicable law. We will verify that person's authority before acting on their request.
File a complaint
If you believe your privacy rights have been violated, you may file a complaint with Lux Family Chiropractic or with the U.S. Department of Health and Human Services Office for Civil Rights.
We will not retaliate against you for filing a complaint.
2. Your Choices
In certain circumstances, you may tell us your preferences about sharing information with family members, close friends, or other individuals involved in your care or payment for your care.
We will follow your preferences when required by law. If you cannot communicate your wishes, we may use professional judgment and applicable legal standards to determine whether sharing is appropriate.
You may also express preferences about disclosures for disaster relief purposes.
We generally must obtain your written authorization for uses or disclosures involving:
Marketing communications that require authorization under HIPAA.
The sale of protected health information.
Most uses or disclosures of psychotherapy notes, if applicable.
Other uses or disclosures not permitted by law or described in this notice.
You may revoke a written authorization at any time by notifying us in writing. Revocation does not undo actions already taken in reliance on a valid authorization.
3. How We Typically Use and Disclose Your Health Information
Treatment
We may use and share your health information to provide, coordinate, or manage your care.
Example: We may share relevant information with another healthcare provider involved in your treatment.
Payment
We may use and share information to bill for services, collect payment, process claims, and carry out other permitted payment activities.
Example: We may provide necessary information to an insurer to process an eligible claim.
Healthcare operations
We may use and share information to operate our practice, evaluate services, manage records, improve care, and conduct other permitted healthcare operations.
Example: We may review patient records to assess the quality of our services.
Appointments and care-related communications
We may contact you about appointments, scheduling, follow-up care, treatment options, and health-related services, as permitted by law.
We may communicate through approved methods, including telephone, text, email, or patient communication systems, while respecting applicable confidentiality requirements and your communication preferences.
4. Other Uses and Disclosures Permitted or Required by Law
Subject to applicable legal conditions and restrictions, we may use or disclose health information:
For public health activities, including certain disease reporting and public health investigations.

These disclosures are subject to applicable safeguards and restrictions. Some situations require patient authorization or other legal conditions before information may be released.
5. Specially Protected Health Information
Some health information is subject to additional protections under federal or Minnesota law. Where a more protective law applies, we will follow that law.
Substance use disorder treatment records
If Lux Family Chiropractic receives or maintains substance use disorder patient records protected by 42 CFR Part 2, additional confidentiality requirements apply.
In particular, Part 2 records may not be used or disclosed in civil, criminal, administrative, or legislative investigations or proceedings against the patient without the patient's written consent or the specific court authorization and other legal process required by Part 2.
Other uses and disclosures of these records will follow applicable Part 2 consent requirements and restrictions.
If Part 2 information is used for fundraising, applicable advance notice and choice requirements will be followed.
Information disclosed to a recipient may, in some circumstances, be redisclosed by that recipient and no longer protected by HIPAA. Special restrictions may continue to apply to Part 2 records and other information protected by law.
